RN Hospice Admissions - Weekends

About the role

Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated Admissions Registered Nurse (RN) to support patients and families throughout Broward County.

Responsibilities

• Serve as the primary nurse responsible for evaluating patients referred for hospice services
• Educate patients and families on hospice care and clearly communicate all components of our continuum of care
• Collaborate with referring physicians and liaisons to coordinate individualized, hospice‑appropriate plans of care
• Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long‑term care facilities, and other healthcare professionals
• Promote and advocate for the hospice philosophy through education and relationship‑building within the healthcare community

Requirements

• Registered Nurse currently licensed in the State of Florida
• Current Basic Life Support Certification
• Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred
• Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus
