Responsibilities
- Provide personal care according to the patient’s needs.
- Carry out the duties assigned to increase the patient’s level of comfort, and to maintain personal hygiene and a safe environment for the patient.
- Observe the patient, report observations and document observations and care performed.
- Document the care provided and the patient’s response to care in accordance with policies and procedures and regulatory standards.
- Provide timely documentation of patient services based on plan of care.
- Adhere to Company policies and procedures and regulatory compliance requirements and directives.
Requirements
- Travel as necessary (up to 50% of the time) to service patients in their residence of care, which includes home, assisted living facilities, long term care facility, and hospital.
- STNA License (Required).
- Driver's License (Required).
- Hospice experience (Preferred).
- Geriatrics experience (Preferred).
- Hospice and Palliative Medicine (Preferred).
