Hospice Social Worker I

About the role

Care Initiatives Hospice is seeking a compassionate Social Worker to provide comprehensive care to patients during their final stages. This role involves meeting patients and families where they feel most comfortable—whether at home, in a long-term care community, assisted living residence, or hospital—to deliver personalized, holistic care that addresses physical, emotional, and spiritual needs. The Social Worker will collaborate closely with family members, caregivers, and the care team to honor patient needs and wishes with compassion, dignity, and respect.

Responsibilities

• Assess patients' psychosocial needs and complete social histories
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support patients and families with adjustment, rights, and orientation
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support patient, family, and community engagement activities
• Promote facility programs and services to referral partners

Qualifications

• Bachelor's Degree in Social Work, required
• Hospice or palliative care experience, preferred
• Two (2) years of verifiable experience working as a Social Worker, preferred
• Current driver's license, automobile liability insurance, and ability to travel between multiple locations within the region (Iowa Falls, Hampton, Eldora, and surrounding areas)
