About the role
This is a dual role, supporting the hospice team by serving as the bereavement coordinator and supporting the holistic therapy program. The Coordinator of Holistic Therapies is a vital part of the hospice interdisciplinary team, where they will work to support the patients and families receiving services from Mozaic Hospice.
Responsibilities
- Providing in person and telephonic support to bereaved individuals and families.
- Utilizing evidenced based techniques and assessments to provide supportive bereavement care.
- Facilitate bereavement support groups.
- Aiding the hospice social worker with as needed evaluations/revisits
- Coordination and timely deliverance of holistic therapies to hospice patients/families, including but not limited to pet therapy, music therapy, volunteer services, and spiritual.
- Actively participate in weekly IDT meetings, lending a psychosocial lens to a patient and family system.
- Actively recruit, engage, orient, and manage a group of volunteers, providing opportunities for education and community engagement.
- Remain knowledgeable and abide by all regulatory requirements for a hospice program.
- Implement innovative programs to provide education to the greater community and recruit volunteers with unique backgrounds.
- Other related duties as assigned.
Requirements
- Education: Master's degree in social work from a CSWE-accredited program.
- Experience: Experience in medical social work, palliative care, hospice care, and/or geriatrics
- Licensure And Certifications: LMSW or LCSW in the State of Connecticut. Valid driver's license and reliable transportation. Maintains all required licensure and continuing education requirements.
