Hospice Social Worker - Burgundy Team

About the Role

Join a compassionate team dedicated to providing comfort and dignity. Suncoast Hospice, a part of Empath Health, is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve in Pinellas County.

Responsibilities

• Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
• Provide individual and family counseling, crisis intervention, and bereavement support.
• Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
• Assist with financial resources, Medicare/Medicaid, and long-term care planning.
• Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
• Support patients and families with advance directives and memorial planning.
• Educate the community on hospice care.

Qualifications

• Minimum one year of social work experience in a healthcare setting (internships may qualify).
• Must hold one of the following Florida licenses or be License eligible (Registered Intern) within 90 days of hire:
  • Social Work (LCSW) or Social Work (RCSWI)
  • Mental Health Counselor (LMHC) or Mental Health Counselor (RMHCI)
  • Marriage and Family Therapist (LMFT) or Marriage and Family Therapist (RMFTI)
• For field-based positions, employees must have reliable transportation, provide proof of valid automobile insurance, and maintain a copy in the employee’s HR file.
