Qualifications
- Bachelor’s degree in social work from an institution accredited by the Council on Social Work Education.
- Current Social Work licensed in the state(s) of practice.
- Five (5) licensure levels: Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), Licensed Clinical Social Worker (LCSW), Social Work Training Certificate (SWTC).
- Ability to pass background, consumer report, and drug test.
Core Responsibilities
- Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
- Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
- Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
