Job duties
- Provide direct patient counseling services
- Support crisis interventions as needed
- Aid patients and families in community resource planning and access
- Help patients and families navigate their ongoing healthcare journey
Qualifications
- Education: Master of Social Work (MSW) required
- Licensure: Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position required; Licensed Independent Social Worker (LISW) preferred
- Experience: One (1) year of experience practicing as a Licensed Social Worker in a hospice/palliative care or similar setting
- Transportation: Reliable transportation to/from care sites/ or work locations
