Responsibilities
- Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
- Provide individual and family counseling, crisis intervention, and bereavement support.
- Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
- Aid with financial resources, Medicare/Medicaid, and long-term care planning.
- Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
- Support patients and families with advance directives and memorial planning.
- Educate the community on hospice care.
Requirements
- Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).
- Licensure: Must hold one of the following Florida licenses or License eligible (Registered Intern) within 90 days of hire:
- Social Work (LCSW) or Social Work (RCSWI)
- Mental Health Counselor (LMHC) or Mental Health Counselor (RMHCI)
- Marriage and Family Therapist (LMFT) or Marriage and Family Therapist (RMFTI)
Qualifications
- Must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.
- Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
