About the role

Catholic Community Health is a nonprofit faith-based organization committed to providing hospice care and support to patients and their families. We are dedicated to redefining end-of-life care by setting the standard for exceptional care that prioritizes one-on-one interactions, lower caseloads, and holistic care.

Responsibilities

  • Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the RN and other members of the interdisciplinary group, either verbally or through documentation in EMR.
  • Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risk for suicide, neglect or abuse, and plans intervention based on evaluation findings.
  • Counsels patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provide crisis intervention when necessary.
  • Coordinates respite stays as per family/caregiver needs, facilitating the review of medical records and all that is included in the preparation of and follow-through of the respite stay.
  • Provides bereavement counseling.
  • Educates patients on, and assists in, preparation of advanced directives and durable power of attorney for healthcare, if not previously executed, and only if the patient is cognizant.
  • Maintains clinical competency.
  • Assists in coordination of care with personnel at nursing facilities or patients’ other living settings.
  • Collaborates with a multidisciplinary team to deliver holistic care.
  • Supports patients and families with personalized, one-on-one interactions.
  • Upholds the values and mission of Catholic Community Health in all interactions.

Requirements

  • BSW, MSW or LCSW required
  • Hospice experience preferred
  • Alignment with the values and mission of Catholic Community Health
  • Ability to work effectively in an environment that is very diverse
  • Must possess a valid Driver’s license, dependable transportation, current vehicle insurance, and a verified safe driving record
  • Strong organizational and time-management skills with the ability to multitask effectively
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively in a fast-paced environment
