Responsibilities
- Develops and maintains therapeutic relationships.
- Provides comprehensive psychosocial assessments.
- Develops and maintains comprehensive plan of care.
- Provides supportive counseling.
- Facilitates smooth transition from care environments.
- Mobilizes community resources.
- Provides effective patient/family/caregiver teaching.
- Supports the death/dying process.
- Collaborates in providing patient care.
- Creates timely and accurate documentation.
- Affirms patient safety.
- Utilizes resources effectively and efficiently.
- Maintains and enhances professional skills.
- Adheres to Bloom standards and continuously improves processes/services.
- 50% of caseload home-based patients
Requirements
- Master's degree in social work from an accredited school and a minimum of 1-3 years of palliative care or hospice experience.
- Demonstrated ability to assess emotional and social needs of patients and their families.
- Excellent communication and interpersonal skills.
- Ability to collaborate with interdisciplinary team in providing patient care.
- Solution-driven, creative, and resourceful problem-solving skills.
- Independent and effective problem-solving skills.
- Skilled in organizing and prioritizing work.
- Attention to detail, accuracy, and follow-through.
- Professional image, positive attitude, enthusiasm, and self-motivation.
- Comfortable maintaining professional boundaries.
- Proficient computer skills.
- Broad understanding of community resources, including financial benefits.
- Awareness and sensitivity to cultural diversity.
- Knowledge of self-care, personal boundaries, abilities, limits, and inner resources.
- Current Colorado driver's license and automobile insurance.
Preferred Qualifications
- Previous hospice/experience.
- LCSW licensure.
- Bilingual abilities.
