About the role

Deliver skilled nursing care in the comfort of patients' homes, creating individualized care plans, administering medications, educating patients and families, and supporting recovery and independence.

Responsibilities

• Deliver skilled nursing care in the comfort of patients' homes
• Create individualized care plans
• Administer medications
• Educate patients and families
• Support recovery and independence

Requirements

• Graduate of an approved nursing education program, BSN preferred
• Licensure as a Registered Nurse in the State of Connecticut
• One (1) year experience as a professional acute care nurse
• Home care experience preferred but not required
• Must have current Driver’s license and reliable transportation
