About the role
Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.
Responsibilities
- Build rapport with patients, families, physicians, facilities, and other interdisciplinary team members.
- Coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.
Requirements
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Current State License as a Registered Nurse.
- Minimum one (1) year experience as a nurse in a clinical care setting.
- Reliable transportation, valid driver's license, and valid automobile insurance coverage.
Qualifications
- Bachelor of Science in Nursing (BSN).
- Hospice or home health experience.
Skills
- Nursing skills as defined by accepted nursing standards.
- Ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment.
- Ability to cope with family emotional stress and tolerance of individual lifestyles.
- Handle multiple priorities, documentation requirements and deadlines.
