Bring your nursing expertise to one of healthcare's most meaningful specialties. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

Responsibilities

• Build rapport with patients, families, physicians, facilities, and other interdisciplinary team members.
• Coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.
• Provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

Requirements

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one (1) year experience as a nurse in a clinical care setting
• Reliable transportation, valid driver's license, and valid automobile insurance coverage

Qualifications

• Bachelor of Science in Nursing (BSN)
• Hospice or home health experience

Skills

• Nursing skills as defined by accepted nursing standards
• Ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Ability to cope with family emotional stress and tolerance of individual lifestyles
• Handle multiple priorities, documentation requirements and deadlines
