About the role

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in various states. The Hospice RN Case Manager position is based in or near Chester and Delaware Counties, PA.

Responsibilities

  • Manage assigned patient caseload and perform assessments, planning, implementation, and evaluation of patient care across all phases of the nursing process.
  • Document all notes in the EMR and submit them accurately and promptly.
  • Communicate and collaborate with attending physicians, hospice staff, and other agencies to coordinate optimal care for patients.
  • Educate and coach relevant parties on best practices for patient care, including physical, mental, and emotional aspects.
  • Participate in interdisciplinary team meetings to address issues, share professional support, and enhance professional growth.
  • Assume the role of associate nurse during off-hours or as requested by the Patient Care Coordinator to support patients and families.
  • Provide support at the time of death and during the bereavement period for family members and loved ones.
  • Attend and participate in Hospice orientation, in-service training, and community programs related to hospice care.
  • Maintain knowledge of agency policies and regulatory requirements governing hospice care.

Requirements

  • Valid State license to practice as a Registered Nurse in the State of Pennsylvania.
  • At least 1-2 years of work experience as a professional nurse, preferably in hospice care.
  • Ability to assess and respond to the needs of patients and loved ones/caregivers in various settings.
  • Skilled in nursing practice, capable of coping with emotional stress and accommodating diverse lifestyles.
  • Able to make meaningful observations, conduct skilled assessments, and write comprehensive, accurate reports.
  • Team player with an interdisciplinary approach to patient care.
  • Valid driver's license, good driving record, reliable vehicle, and ability to drive year-round.
  • Able to follow directions, work independently, and interact positively with a multidisciplinary team.

Pay

Starting at $90,000 salary for full-time employees.
