Responsibilities

  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs, collaborating with the interdisciplinary team to develop personalized care plans.
  • Care for and coordinate patient care, ensuring interventions align with patients' wishes, goals, and the hospice philosophy.
  • Provide skilled nursing care, pain and symptom management, and emotional support to patients and their families in accordance with established protocols.
  • Collaborate with physicians, social workers, chaplains, and other team members to address the diverse needs of patients and families.
  • Communicate effectively with patients, families, and the care team, providing education about the hospice process and supporting informed decision-making.
  • Maintain oversight of patients' condition, updating care plans as necessary, and ensuring timely documentation in electronic medical records.
  • Participate in regular interdisciplinary team meetings, sharing insights and expertise to improve patient care delivery.
  • Engage in continuous education and training to maintain and enhance clinical skills and knowledge.
  • Actively participate in team events, fostering a collaborative and supportive work environment.
  • Contribute to the recruitment incentive program by referring potential team members who align with our values and mission.
  • Adhere to ethical and professional standards, maintaining confidentiality and upholding the hospice's reputation for excellence.

Qualifications

  • Current Registered Nurse (RN) license in the state of practice.
  • Hospice or Home Health experience preferred.
  • Strong communication and interpersonal skills, with the ability to empathize and connect with patients and families.
  • Proficiency in electronic medical records and documentation.
  • Ability to work effectively within an interdisciplinary team.
  • Commitment to providing patient-centered and compassionate care.
  • Comfortable with travel and a flexible schedule.
