About the role

The RN Case Manager - Hospice plays a crucial role in providing quality hospice care, ensuring compliance with company standards and regulations. They are responsible for assessing patient needs, developing and implementing comprehensive care plans, communicating with healthcare providers, and overseeing the implementation of the patient's Plan of Care.

Responsibilities

  • Provides ongoing assessment and nursing care to patients in homes and health care facilities.
  • Ensures an accurate evaluation of patient needs and develops and implements a comprehensive care plan.
  • Fulfills current nursing professional standards of practice and agency-approved hospice protocols.
  • Manages symptom management to ensure patient comfort.
  • Communicates with other healthcare providers regarding changes in patient condition and needs.
  • Affords education to patients and caregivers as needed.
  • Oversees the implementation of the patient's Plan of Care and evaluates its effectiveness on an ongoing basis.
  • Prepares for IDG by reviewing the Plan of Care, reconciling medications, and projecting patient/family needs.

Requirements

  • Current unencumbered license to practice as a Registered Nurse (RN) in the state of Ohio.
  • One year of experience in acute care or medical-surgical nursing.
  • Experience in hospice care is preferred.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
  • Proficiency with electronic medical records (EMR) is preferred.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess a valid driver's license and acceptable driving record.
  • Must be insurable under professional liability and crime coverage policies as specified by the insurance carrier.
  • Must be able to sit, stand, walk, lift up to 50 pounds, drive up to 6 hours per day, work weekends, evenings, and holidays, and be on-call for emergencies.
  • May be exposed to infectious waste, disease, conditions, and exposure to the AIDS and Hepatitis B viruses.
  • Must be subject to the risk category for exposure to bloodborne diseases.

Qualifications

  • Education: Current unencumbered license to practice as a Registered Nurse (RN) in the state of Ohio required.
