About the Role

Join Care Initiatives Hospice as an RN Case Manager and provide comprehensive, compassionate care to patients during their final stages. You'll meet patients and families where they feel most comfortable—whether at home, in a long-term care community, assisted living residence, or hospital—delivering personalized care that addresses their unique physical, emotional, and spiritual needs. This role involves collaborating closely with family members, caregivers, and an interdisciplinary team to honor patients' wishes and ensure dignity and respect throughout their care journey.

This position serves patients in Storm Lake, Pocahontas, Sac City, and surrounding areas.

Responsibilities

  • Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high-quality patient care in the patient’s place of residence.
  • Develop and oversee the plan of care under physician direction, ensuring appropriate interdisciplinary involvement for optimal patient outcomes.
  • Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
  • Conduct continuous visits and interdisciplinary assessments of the patient.
  • Deliver quality care and complete clear, concise, and accurate clinical documentation.
  • Provide patient, caregiver, and family counseling and education as indicated in the plan of care.
  • Develop and continually update the plan of care to meet patient-specific needs.

Requirements

  • Active Iowa RN license.
  • Active CPR certification.
  • Hospice or palliative care experience.
  • Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
  • Current driver’s license, automobile liability insurance, and ability to travel between multiple locations within your region.
  • Home access to the internet.
  • Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
