Hospice Resource RN - PM Shift (Sun Prairie, Middleton Area)
Location: Sun Prairie/Middleton, WI | 0.8 FTE (32 hours/week) | PM Shift | Weekdays 1600-0030 | Includes on-call rotation (1 NOC shift/month), every other weekend, and holiday rotations (3/year).
About the role
Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. As a Registered Nurse (RN) on the Alternate Shift, you will provide meaningful, patient-centered care by conducting assessments, triaging patient needs, creating and updating plans of care, delivering interventions, and ensuring effective communication and documentation. This role covers care groups across homecare teams during vacancies, FMLA, or high census periods.
Responsibilities
- Patient and Family Assessments:
- Assess physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs as outlined in the plan of care (POC).
- Collect and document data from patients, families, and interdisciplinary team members.
- Identify, interpret, and report significant changes in condition or unusual incidents (e.g., abuse/neglect, suicidal intent, harmful coping mechanisms).
- Collaborate with the interdisciplinary team (IDT) and primary care providers to determine hospice eligibility.
- Patient Visits:
- Respond to patient and family needs, including changes in condition or requests for acute intervention.
- Provide interventions, treatments, and pain management tailored to patient and family needs.
- Evaluate and adjust pain management or other interventions for effectiveness.
- Educate patients and families on pain/symptom management, end-of-life care, and other topics, adjusting education as needed.
- Plan of Care:
- Formulate, implement, and revise individualized POCs in collaboration with the IDT, patient, and family.
- Include goals, pain management, interventions, education, and discharge plans in the POC.
- Establish achievable outcome measures and prioritize care provision.
- Document interventions, outcomes, and changes in patient condition in the electronic health record (EHR).
- Delegate visits to appropriate disciplines and oversee the work of Certified Nursing Assistants (CNAs) and Licensed Practical Nurses (LPNs).
- Miscellaneous:
- Participate in meetings to plan and evaluate patient/family services.
- Precept new staff and students.
Requirements
- Current, unrestricted Registered Nursing License in the state of employment.
- One year of nursing experience (preferred).
- Valid driver’s license, auto insurance, and access to a reliable vehicle.
- Basic Microsoft Office skills and intermediate electronic health record (EHR) skills.
- Ability to use proper body mechanics, safe patient handling, and standard precautions.
- Must be at least 18 years of age.
