- Shift: PM Shift | Weekdays | 4pm–12:30am
- On-call rotation: 1 NOC Shift/Month
- Weekend rotation: Every other weekend
- Holiday rotation: 3 per year
- Sign-on bonus: $10,000
About the role
Join Agrace, a nationally recognized nonprofit leader in hospice and palliative care. The Registered Nurse Alt Shift provides patient and family-centered care consistent with Agrace philosophy, goals, standards, regulations, and policies. This role provides coverage for care groups across homecare teams when needs arise (vacancies, FMLA, high census).
Key responsibilities
- Patient and Family Assessments: Assess patients' and families' physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs as outlined by the plan of care (POC). Collect data from patients, families, significant others, interdisciplinary team members, and other health care professionals. Identify, interpret, and document assessment data collected in nursing interviews, physical assessments, and other health data. Document and report significant changes in condition or unusual incidents (e.g., client abuse/neglect, suicidal intent, harmful coping mechanisms, violence) to appropriate IDT members. Collaborate with IDT and primary care providers to create HES for hospice eligibility determination.
- Patient Visits: Respond to patient and family needs or changes in condition requiring acute intervention. Provide interventions, treatments, and pain management to meet needs. Evaluate effectiveness of pain management or other interventions and adjust as needed. Provide patient and family education on pain, symptom management, end-of-life care, and other topics. Evaluate effectiveness of education and adjust to meet needs.
- Plan of Care: Formulate, implement, and revise individualized POC in collaboration with the Interdisciplinary Team (IDT), patient, and family. The POC includes goals, pain management, interventions, education, discharge plan, and more. Develop reasonable and achievable outcome measures and establish priorities in planning and care provision. Read, understand, and follow the plan of care. Document interventions, outcomes, and changes in patient condition in the electronic health record (EHR). Delegate visits to appropriate disciplines and manage visit sets. Oversee work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN).
- Miscellaneous: Participate in meetings to plan and evaluate patient/family services. Precept new staff and students.
Qualifications
- Current, unrestricted Registered Nursing License in state of employment
- One year of nursing experience (preferred)
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Must be at least 18 years of age
- Basic Microsoft Office skills
- Intermediate electronic health records skills
- Ability to use proper body mechanics, safe patient handling, and standard precautions
