About the Role
Agrace, a nationally recognized nonprofit leader in hospice and palliative care, is seeking a skilled CNA who values meaningful, patient-centered care. This role involves providing patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.
Responsibilities
- Assist patients with activities of daily living including ambulation, changing position, moving to/from sitting and standing, toileting and incontinence care, bathing, showering, dressing, oral care, grooming, eating and drinking
- Perform post mortem care, obtain vital signs, assist with treatments, and anticipate and respond to patient needs
- Provide homemaker assistance including light housekeeping such as changing linens, tidying rooms, doing laundry, removing trash, ordering and delivering patient supplies, and helping prepare simple meals
- Read, understand, and follow the plan of care; document cares and changes in patient condition in electronic health record
- Document and report significant change in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence
- Reinforce patient, family, and facility partner education
- Participate in meetings to plan and evaluate patient/family services
- Precept new staff and students; delegate tasks to volunteers as appropriate
Qualifications
- High school diploma or GED, or equivalent combination of education and work experience
- Graduation from an accredited Certified Nursing Assistant program
- Current, unrestricted Nursing Assistant Certification in state of employment
- Valid driver's license, auto insurance, and access to a reliable vehicle
- Basic Microsoft Office skills
- Experience using electronic health records; Epic preferred
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions
- One-year personal care experience, hospice, or home health, preferred
Pay & Schedule
- 0.7 FTE (28 hrs/week) | Day Shift | Monday-Friday | 8:00am-4:30pm
- No weekends and holiday (3/year) rotations
- Starting Pay: $18.50+ (based on experience)
- Sign on bonus: $3,000
