Responsibilities

• Delivery of Patient Care:
  • Coordinate and deliver patient/caregiver care
  • Conduct nursing assessments to assist in the overall management of patient/caregiver needs
  • Develop, implement, evaluate, and update the plan of care to ensure uninterrupted care and achievement of goals
  • Facilitate patient and caregiver independence
  • Utilize VIA Health Partners patient-related protocols, standing orders, and guidelines for hospice care
  • Conduct effective patient and caregiver education, allowing time for questions and consultation
  • Communicate effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care
  • Maintain open lines of communication with supervisor regarding complex patient/caregiver issues
  • Apply principles of infection control and follow guidelines for universal precautions
• Interdisciplinary Group:
  • Attend, participate, and may facilitate interdisciplinary group conferences (IDG) and special care conferences
  • Establish and maintain effective working relationships with team members
  • Identify specific patient/caregiver needs and make referrals to appropriate team members
• Documentation:
  • Ensure accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and licensure/certification standards
• Professional and Personal Accountability:
  • Demonstrate competency in knowledge and skills
  • Maintain and project VIA Health Partners' professional reputation
  • Stay abreast of current trends in the healthcare field and incorporate new information and methods into practice
  • Maintain current professional licensure, CPR certification, and CEU requirements
  • Maintain productive and professional relations with external agencies and healthcare providers
  • Exhibit time management skills, planning workdays while remaining responsive to crises
• Inpatient Unit (IPU) Duties:
  • Clean patient rooms as required per patient room cleaning procedure
  • Ensure all areas of the facility are clean and appropriately stocked with supplies
  • Maintain cleanliness of the family room throughout the day
  • Use proper chemicals for cleaning different surfaces
  • Use protective equipment when working with chemicals (e.g., gloves, glasses)
• Perform other duties as required

Requirements

• A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred
• A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required
• Current CPR certification required; CHPN preferred
• Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions
• Experience in direct care of patients with life-limiting illnesses required (e.g., hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, medical-surgical)
• Computer proficiency is required
