Responsibilities
- Delivers patient/caregiver care by coordinating and managing care.
- Conducts nursing assessments to manage patient/caregiver needs.
- Develops, implements, evaluates, and updates the plan of care.
- Facilitates patient and caregiver independence.
- Applies VIA Health Partners patient-related protocols, standing orders, and guidelines.
- Provides patient and caregiver education, addressing questions and consultations.
- Communicates with interdisciplinary groups and other healthcare professionals.
- Maintains open lines of communication with supervisors and reports on complex patient/caregiver issues.
- Follows infection control principles and guidelines for universal precautions.
- Attends and participates in interdisciplinary group conferences and special care conferences.
- Establishes and maintains effective working relationships with team members.
- Identifies and refers patient/caregiver needs to appropriate team members.
- Ensures accurate, complete, succinct, and timely clinical documentation.
- Maintains professional and personal accountability by demonstrating competency, upholding the organization's reputation, and engaging in professional development.
- Performs IPU duties such as cleaning patient rooms, ensuring cleanliness and stock of supplies, and maintaining the family room.
Requirements
- A Diploma or associate degree in nursing is required; a Bachelor’s Degree is preferred.
- A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
- CPR certification is required; CHPN certification is preferred.
- Minimum 1 year of related experience in inpatient units (IPU) and 2 years in other nursing positions.
- Experience in direct care of patients with life-limiting illnesses is required.
- Computer proficiency is required.
Qualifications
- Current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN).
- Experience in direct care of patients with life-limiting illnesses.
- Related experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical surgical fields.
