Responsibilities

  • Delivers patient/caregiver care by coordinating and managing care.
  • Conducts nursing assessments to manage patient/caregiver needs.
  • Develops, implements, evaluates, and updates the plan of care.
  • Facilitates patient and caregiver independence.
  • Applies VIA Health Partners patient-related protocols, standing orders, and guidelines.
  • Provides patient and caregiver education, addressing questions and consultations.
  • Communicates with interdisciplinary groups and other healthcare professionals.
  • Maintains open lines of communication with supervisors and reports on complex patient/caregiver issues.
  • Follows infection control principles and guidelines for universal precautions.
  • Attends and participates in interdisciplinary group conferences and special care conferences.
  • Establishes and maintains effective working relationships with team members.
  • Identifies and refers patient/caregiver needs to appropriate team members.
  • Ensures accurate, complete, succinct, and timely clinical documentation.
  • Maintains professional and personal accountability by demonstrating competency, upholding the organization's reputation, and engaging in professional development.
  • Performs IPU duties such as cleaning patient rooms, ensuring cleanliness and stock of supplies, and maintaining the family room.

Requirements

  • A Diploma or associate degree in nursing is required; a Bachelor’s Degree is preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • CPR certification is required; CHPN certification is preferred.
  • Minimum 1 year of related experience in inpatient units (IPU) and 2 years in other nursing positions.
  • Experience in direct care of patients with life-limiting illnesses is required.
  • Computer proficiency is required.

Qualifications

  • Current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN).
  • Experience in direct care of patients with life-limiting illnesses.
  • Related experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical surgical fields.
