About the Role
Provide end-of-life hospice care and support to patients in their homes.
Responsibilities
- Deliver compassionate, patient-centered care
- Perform assessments, symptom and pain management
- Develop individualized care plans
- Educate and support patients and families
- Ensure thorough EMR documentation
- Work independently with travel within assigned territory
Requirements
- Active RN license in the state
- Minimum of two years of RN experience (hospice experience preferred)
- Valid driver’s license
- Ability to travel approximately 20% of the time
