Hospice Registered Nurse (RN)

Responsibilities

• Provide full nursing care for hospice patients, developing and implementing personalized Plans of Care in collaboration with healthcare professionals.
• Conduct initial evaluations within 48 hours of referral to determine eligibility.
• Assess and re-assess patients' physical, psychosocial, and environmental needs, updating care plans at least every 15 days.
• Document patient problems, goals, interventions, and responses to care.
• Collaborate with patients, families, physicians, and interdisciplinary team members to ensure comprehensive care.
• Educate patients and families on self-care techniques when appropriate.
• Maintain accurate clinical notes regarding patient conditions.

Qualifications & Skills

• Unrestricted RN license in the practice state.
• At least 1 year of clinical experience.
• Current CPR certification and valid driver's license with reliable transportation.
• Ability to work effectively in diverse home environments.
• Preferred: Ability to work independently with strong communication, organizational, and writing skills.

Compensation & Benefits

Salary ranges from $71,640 to $107,460 annually, with a $5,000 sign-on bonus for full-time nurses.
