Essential Duties
- Administer skilled nursing care for clients of all ages in their place of residence.
- Carefully assess the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs and Activities of Daily Living (ADLs).
- Implement the individualized Plan of Care (POC) and recommend revisions as necessary.
- Consult with and educate the patient and family about the disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
- Train other staff as needed.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Prepare clinical and progress notes demonstrating progress toward established goals.
- Coordinate all patient and family services and prioritize needs with the interdisciplinary team.
- Use a case management approach and refer to other services as needed.
- Inform physicians and other personnel of changes in the patient's needs and outcomes of interventions, while evaluating the patient's and family's response to care.
- Determine the scope and frequency of services needed based on the patient's acuity and family needs.
- Assess the caregiver's ability to meet the patient's immediate needs upon admission and throughout care.
- Evaluate own needs for support and utilize identified systems to meet those needs.
- Communicate information using current processes and technology available to the organization.
- Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
- Perform on-call responsibilities and provide on-call service to patients and families as assigned.
Qualifications
- Graduate of an accredited Associate or Bachelor’s degree in Nursing.
- Current state license as an RN and current state driver’s license.
- Two years' experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
- Current CPR certification.
- Proficient skill using a computer is required; Microsoft Office Suite experience is preferred.
- Strong interpersonal skills and ability to provide effective support to the patient and family.
- Demonstrate skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
- Strong communication and organizational skills and high standard of ethics and compliance.
