Hospice Registered Nurse RN

About the role

Responsible for providing compassionate care to patients and families in their homes. Works closely with healthcare providers to coordinate care plans.

Responsibilities

• Provide hospice care services including medication administration, wound care, and symptom management.
• Coordinate with healthcare providers to ensure comprehensive patient care plans are implemented.
• Communicate regularly with patients and families to monitor their needs and adjust care plans as necessary.
• Document all interactions and care provided in accordance with hospice policies and regulations.

Requirements

• Bachelor's degree in a related field or equivalent experience.
• Current CPR certification.
• Experience in home health care or hospice preferred.
• Strong communication and interpersonal skills.

Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work flexible hours including evenings and weekends.
