Hospice Registered Nurse (RN) – 7 On / 7 Off Schedule

Responsibilities

• Thoroughly assess patients and their families to identify home care needs.
• Provide comprehensive physical assessments and document patient histories.
• Develop and update individualized care plans with active patient and family participation.
• Identify and address discharge planning needs before patient discharge.
• Offer counseling to patients and families on nursing and related health needs.

Requirements

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS) certification, including hands-on CPR.
• Valid driver's license and auto insurance.
• A minimum of an Associate's or Bachelor’s degree in Nursing, with 1 year of home health experience preferred.

• Experience with electronic health records (EHR).

Pay

The pay range for this position is $50,000 - $80,000 annually, depending on experience and qualifications.

Schedule

This position is available on a 7 On / 7 Off schedule.
