Hospice Registered Nurse - Full Time

Role Overview: The Hospice Registered Nurse provides comprehensive pain management and symptom control to hospice patients, ensuring quality end-of-life care in collaboration with an interdisciplinary team, attending physicians, and the hospice Medical Director. Key Responsibilities Delivers timely hospice services aligned with the patient’s care plan, assessing patient and family needs during home visits, and documenting assessments, interventions, and outcomes.Coordinates patient care with team members, healthcare providers, and facilities, ensuring continuity and effective communication.Participates in developing, reviewing, and revising the patient’s plan of care, evaluating intervention effectiveness, and documenting progress.Supervises care provided by aides and practical nurses, ensuring adherence to care plans and proper documentation.Facilitates care delivery in long-term care facilities, attending care meetings, and coordinating with facility staff.Provides ongoing education to patients and families, monitors patient eligibility for hospice, and completes certification documentation.Participates in on-call duties, quality improvement, infection control, and community education efforts.Maintains compliance with all regulatory standards, documents accurately, and upholds privacy and security protocols.
