Hospice Registered Nurse

About the role

We are searching for a compassionate, dynamic, energetic, organized, and efficient Full-Time Hospice RN with hospice experience to join our team in the Lansing and surrounding areas. This role involves providing one-on-one nursing care, building close relationships with patients, and delivering individualized care plans tailored to their needs. You will offer emotional support to help patients live their final days with dignity and comfort, conduct supervisory visits, and participate in interdisciplinary group meetings to develop and revise patient/family care plans.

Responsibilities

• Initial and ongoing assessment of patient's physical, functional, psychosocial, and environmental needs
• Implement and revise individualized plans of care as needed
• Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare requirements
• Consult with and educate patients and families on disease processes, self-care techniques, end-of-life care, and ethical concerns
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes and participate in interdisciplinary group (IDG) conferences
• Engage in the Hospice performance improvement program
• Maintain a high level of customer satisfaction with clients, families, and caregivers
• Communicate information effectively using current processes and technology within the organization
• Participate in field visits and weekly office meetings as indicated
• Adapt to patient needs on a full-time, on-call basis

Requirements

• Current Michigan RN license in good standing
• Minimum of 1 year of hospice experience
• Empathetic approach to patients and caregivers
