Provides compassionate care to patients and families in their homes, focusing on symptom management and emotional support.

Responsibilities

• Assesses patient needs and develops personalized care plans
• Administers medications and treatments as prescribed
• Facilitates communication between patients, families, and healthcare providers
• Maintains accurate records and reports

Requirements

• Bachelor's degree in a healthcare-related field
• Current state licensure as a hospice aide or equivalent
• Experience in home health care or related field preferred

Qualifications

• Strong interpersonal skills
• Excellent organizational and time management abilities
• Ability to work independently and as part of a team
