Hospice On-Call LVN - 7 on / 7 off

About the role

At Brighton/Suncrest Hospice our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Responsibilities

• Provide care and services for a group of hospice patients, visiting patients in private homes, assisted living facilities, and hospitals.
• Work on an after‑hours on‑call schedule: seven days/nights on and seven nights/days off.

Requirements

• Current LVN licensure (in good standing) in the state(s) of practice.
• At least one year of experience as an LVN under license.
• On‑call experience preferred.

Skills

• Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Strong one‑on‑one patient interaction while receiving support from a patient‑care team.
• Independent work skills, including phone, email, written communication, and meetings with various healthcare professionals.
• Understanding of hospice rules and regulations.
• Time‑management skills to prioritize visits, documentation, meetings, and urgent needs.
• Ability to learn and use an electronic medical records system.
• Reliable transportation, valid driver’s license, and auto insurance for travel across the service area.
