About the Role

Responsibilities
- Serve as a proactive, collaborative team member in a hospice care environment.
- Conduct comprehensive adult/geriatric assessments of hospice patients and evaluate and manage the plan of care.
- Demonstrate a commitment to caring for patients and partners.
- Complete all documentation and records in a timely and accurate manner.
- Follow established policies and procedures as related to the scope of practice.
- Maintain continuity of patient care in a hospice setting.
Requirements
- Current, active, and unrestricted Registered Nurse (RN) and Nurse Practitioner (NP) licensure in the state of practice.
- Board-certified by AANP or ANCC in adult, family practice, geriatric, or adult-gerontology primary care.
