About the role

As a Hospice Nurse Practitioner, you’ll contribute to our mission by providing compassionate and dedicated care. This role involves completing routine and emergency “face to face” recertification visits, communicating with medical directors and interdisciplinary groups, and ensuring patient and family needs are met in the home to prevent unwanted hospitalizations.

Responsibilities

• Completes routine and emergency “face to face” recertification visits with patients entering their 3rd benefit period or beyond, including admissions, transfers, and extended length of stay.
• Records observations, treatments, and other pertinent information.
• Communicates with Medical Director, Attending Physician, branch leadership, and IDG regarding findings from visits and measures to alleviate symptoms.
• Provides physical and emotional care to support the patient and family in periods of crisis.
• Triages consults by patient acuity.
• Performs bedside palliative care with patient/family education.
• Ensures all activities align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
• Complies with applicable laws and regulations regarding Collaborative Agreements.
• Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits.
• Provides training and continuing education for staff.
• Assists in the development of clinical practice guidelines/standards in support of quality palliative care.
• Adheres to the practice of confidentiality regarding patients, families, staff, and the organization.
• Collaborates and communicates with Medical Director, attending physician, branch leadership, and IDG.
• Performs other duties/projects as assigned.

Requirements

• Master’s degree in nursing and certification through a national Nurse Practitioner Licensing board.
• 1+ years of clinical nursing experience.
• 1+ year post-graduate clinical nursing experience in the hospice and palliative care field preferred.
• Prior experience in Family Medicine, Geriatrics, and psychiatry preferred.
• Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations.
• Maintain active DEA certification if state allows prescription of controlled substances by APPs.
• 50% travel required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and insurance, and reliable transportation to perform job tasks.
