Hospice Nurse Practitioner

About the Role

Provide palliative, symptom management, and supportive care to hospice patients.

Responsibilities

• Conduct face-to-face patient visits.
• Collaborate with physicians to develop and coordinate care plans.
• Document clinical findings and maintain accurate clinical notes.
• Communicate patient status to the medical team and administrators.
• Educate patients and families on self-care techniques.

Requirements

• Active unrestricted license as a Nurse Practitioner or Nurse Specialist.
• Current national certification (e.g., ANCC, NBCHPN, AANP).
• CPR certification.
• At least 5 years of nursing experience.
• Valid driver’s license and reliable transportation or access to public transit.

Preferred Skills

• Experience in oncology, hospice, geriatrics, or intensive care.
• Palliative care certification.
• Teamwork experience within interdisciplinary settings.

Pay

Salary range from $109,500 to $164,000 plus bonuses.
