Compass Healthcare Consulting & Placement is conducting a search for a Hospice Medical Social Worker position making visits to patients at home in Brooklyn, NY. The opening is available within Brooklyn, NY.

Responsibilities

• Provide medical social services, psychosocial and bereavement counseling to hospice patients and families at home.
• Travel to patient locations; requires a valid driver’s license and access to a vehicle.
• Serve patients throughout Brooklyn, NY.

Requirements

• New York State driver’s license required.
• Ability to travel to patient locations and access to a vehicle.

Qualifications

• MSW from an accredited school of social work.
• LMSW or LCSW required.
• Minimum one (1) year bereavement counseling experience preferred.
• At least one year of Hospice or Home Care experience preferred.
• One year related experience (in addition to education).

Skills

• Excellent interpersonal and communication skills.
• Ability and attitude relative to the spirit of caring as required by specific job duties and functions.
