About the role

Provide compassionate, clinical psychosocial care that helps patients and families navigate the emotional, social, and practical challenges of end-of-life illness. As a Hospice Medical Social Worker, you'll support individuals facing terminal diagnoses by offering counseling, guidance, and resources that promote dignity, clarity, and emotional well-being during one of life’s most difficult transitions.

Responsibilities

  • Complete comprehensive psychosocial assessments
  • Provide crisis intervention and counseling
  • Support advance care planning conversations
  • Collaborate closely with nurses, physicians, and other hospice professionals
  • Ensure emotional, cultural, and practical needs are fully integrated into each patient’s plan of care
  • Directly support both patients and families while strengthening the care experience across the entire hospice journey

Requirements

  • Master's or doctoral degree from a CSWE-accredited school of social work
  • Current State License as a Master's Social Worker (MSW/LMSW/LCSW)
  • Minimum 1 year of experience as a licensed Master's Social Worker
  • Reliable transportation, valid driver’s license, and valid automobile insurance coverage
  • Willingness to travel to homes, facilities, and community settings

Qualifications

  • Hospice or palliative care experience
  • Clinical experience with grief counseling and end-of-life issues
  • Knowledge of Medicare hospice benefit and Conditions of Participation

Pay Range

$70,000 - $75,000 USD
