HOSPICE MEDICAL SOCIAL WORKER

About the role

Liberty Hospice understands the unique needs of its patients and families facing terminal illness. It provides state-of-the-art care and pain management services, delivered by specially trained staff emphasizing strength, dignity, and compassion.

Responsibilities

• Provide direct patient care services and documentation of patient care data.
• Provide clinical assessment for the development of individualized care plans and set treatment goals.
• Apply a working knowledge of psycho-social principles and family dynamics.
• Identify and assess social, emotional, financial, and environmental factors affecting medical treatment and desired outcomes.
• Communicate with agency personnel and coordinate with other services to provide optimal care.
• Assist with homecare visits as needed.
• Relate well to patients and families at all levels, responding professionally to verbal and nonverbal communication.
• Utilize community resources to provide patient services and educate patients and families on utilizing these resources.
• Evaluate bereavement needs of families through initial bereavement assessments and interdisciplinary team coordination.
• Provide bereavement support and grief care to patients and caregivers.

Requirements

• Master of Social Work (MSW) degree from a Council on Social Work Education-accredited school with one year of experience or experience in a healthcare setting.
• Bachelor of Social Work (BSW) degree from an accredited school of social work with one year of experience or experience in a healthcare setting.
• Bachelor’s degree in psychology, sociology, or another field related to social work, supervised by an MSW, with one year of experience in a healthcare setting.
• Bachelor’s degree from a school of social work accredited by the Council on Social Work Education, employed by the hospice before December 2, 2008, and not required to be supervised by the MSW, with one year of experience in a healthcare setting.
• Strong interpersonal skills, ability to effectively communicate within a multidisciplinary setting.
• Knowledge of computer skills and the ability to learn Microsoft Word, Excel, and use of company-approved documentation program.
• Ability to work autonomously with skills in organization, timeliness, and providing effective care for hospice patients and the family unit.
• Working knowledge of psychosocial principles and family dynamic development.
• Knowledge of identifying and reassessing social, emotional, financial, and environmental factors affecting medical treatment and desired outcomes.
• Knowledge of the stages and reactions to anticipatory grief and grief work.
• Participation as an active member of the interdisciplinary team.
• Valid driver's license.
