Hospice LPN - Weekends PRN

About the role

Provide compassionate, patient-centered end-of-life care in a hospice setting, collaborating with an interdisciplinary team to ensure comfort, dignity, and quality of life. Conduct ongoing patient assessments during evenings, nights, and weekends, focusing on symptom management and changes in condition, escalating concerns to RNs as needed. Educate and support patients and caregivers on care plans and symptom relief strategies. Work independently while traveling to patient homes within your territory.

Responsibilities

• Deliver compassionate care supporting patient comfort and dignity
• Provide after-hours assessments and support
• Reinforce end-of-life care plans with patients and caregivers
• Maintain accurate, compliant EMR documentation
• Travel to patient homes within assigned territory (approximately 20% of time)

Requirements

• Active LPN license in the state of practice
• Minimum of two years of LPN experience (hospice experience preferred)
• Current driver’s license
