Hospice LPN
About the role
As a Hospice Licensed Practical Nurse (LPN) you will be called to care when you're needed most. As part of Interim HealthCare, you'll support a full range of patient services to bring comfort and dignity to our clients.
Responsibilities
- Present the Hospice benefit to patients and their families/representatives
- Assess signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
- Follow established plan of care while collaborating and communicating with RN Case Manager and Interdisciplinary Team, including physician, as appropriate
- Communicate with patients' family or friends to provide advice, comfort, and release instructions
Requirements
- Current license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Valid state-issued driver's license, automobile insurance, and reliable transportation; willingness to travel
- One (1) year preferred experience practicing as a Licensed Practical Nurse (LPN) in a hospice or similar setting
