We are seeking driven, compassionate LPNs who take pride in the quality of their care and services. This full-time hospice LPN position supports our rapid expansion throughout Indiana.

Responsibilities

• Provides skilled and palliative nursing in the home as ordered on the plan of treatment and nursing care plan.
• Evaluates clinical data findings and communicates pertinent information about the patient’s clinical status and ongoing needs to others in a timely fashion.
• Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Adequately evaluates and re-evaluates pain, utilizing appropriate pain management techniques.
• Educates the patient and family regarding pain management and notifies the RNCM when pain is not manageable on current medications.
• Demonstrates knowledge of medications and their correct administration based on the age of the patient and the patient’s clinical condition.

Requirements

• Licensed Practical Nurse in the State of Indiana.
• Proof of current classroom CPR certification.
• Hepatitis profile and TB vaccination record.
• Current driver’s license, reliable transportation, and insurance.

Schedule

Monday through Friday, 8:00 AM – 5:00 PM.

Available territory openings: LPN to cover patients in 1-2 counties surrounding the office.
