Licensed Practical Nurse (LPN) Responsibilities

• Deliver direct patient care: Provide skilled nursing care including wound care, medication administration and pain/symptom management under the supervision of an RN Case Manager.
• Be the Patient’s Advocate: Support each patient’s unique needs, preferences, and wishes while providing compassionate guidance to their loved ones.
• Build Meaningful Connections: Develop trusting relationships with patients and families, offering emotional support and reassurance when it’s needed most.
• Contribute to Care Planning: Assist in implementing and updating individualized care plans that prioritize comfort and quality of life.
• Document with Care: Accurately record patient progress, observations, and care activities to support continuity of care.

Required Qualifications

• Active LPN License in the state of Ohio
• Valid Driver’s License
• 1+ year of experience in hospice, palliative care, or a related clinical setting

Preferred Qualifications

• BLS Certification
• Hospice and Palliative Medicine background
• Experience working under RN supervision in a home health or hospice environment
