Hospice LPN (37607)

Job Summary

The LPN supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Mishawaka, IN
• RN Case Managers may participate in an on-call rotation with additional pay
• Some branches offer dedicated on-call staff, resulting in backup on-call coverage only

Qualifications

• Active Licensed Practical Nurse (LPN) license
• Hospice or home health experience preferred
• Strong clinical assessment and care coordination skills
• Strong communication and documentation skills
• Experience using HCHB (Homecare Homebase) preferred

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
