About the role

At Envision Hospice, we provide continuous care for hospice patients in various settings such as private homes, adult family homes, assisted living communities, and skilled nursing facilities. The role involves providing comprehensive support to patients and their families, collaborating with the interdisciplinary care team, and adhering to hospice policies and procedures.

Responsibilities

  • Follow all hospice policies, procedures, and care protocols.
  • Implement care plans under the direction of the RN and IDG team.
  • Provide assigned nursing care, treatments, and preventive procedures.
  • Maintain accurate and timely clinical documentation.
  • Attend staff meetings, IDG conferences, and required training.
  • Communicate schedule changes and contribute to interdisciplinary care planning.
  • Assist with procedures, maintain equipment, and observe proper infection control.
  • Support patient and family self-care techniques.

Requirements

  • Current, active Washington State LPN license in good standing.
  • Active BLS certification.
  • Valid driver’s license and reliable transportation.
  • Minimum 1 year nursing experience in hospice or home health preferred.
  • Minimum 2-3 years of healthcare related experience required.
  • Proficient with EMR systems and healthcare technologies.
  • Ability to exercise sound judgment, initiative, and work independently within scope of practice.
  • Adaptable to diverse social, cultural, and religious backgrounds.

Schedule

  • Continuous Care shifts typically range from 8 to 12 hours.
  • Full-time hours may fluctuate based on patient census, eligibility, and care needs.
  • Schedule may include days, evenings, nights, weekends, and holidays.
  • Flexibility and availability for urgent patient needs are required.
