About the Role
Hospice of Marion County, a part of Empath Health, is seeking a caring and motivated LPN to join our Patient Care Team to visit hospice patients in facilities in Marion County.
Schedule
Monday – Friday, 8am to 5pm.

Responsibilities
- Provide ongoing monitoring of physical, psychological, social, and spiritual needs of the Patient/Family/PCG and initiate appropriate intervention or referral in collaboration with Primary RN.
- Provide accurate and appropriate documentation of patient/family services as provided for in Empath Health policies and submit in a timely manner.
- Provide nursing care under the direction of the Primary RN to a specified group of patients to ensure continuity of care. Participate in weekly review and update of care plans in collaboration with Primary RN.
- Receive and implement orders under the direction of the RN from the physician.
- Following a patient’s death, collaborate with primary RN/Counselor and participate as needed in closure contact with family/caregivers.
- Demonstrate knowledge of Empath Health policies and procedures and pertinent State and Federal Rules and Regulations.
- Administer medications and treatments prescribed by the attending physician in collaboration with Primary RN and/or PFCC.
Requirements
- Valid Florida LPN License.
- Valid Florida State Driver’s License.
- Valid CPR certification from the American Red Cross or American Heart Association.
- Current IV Therapy certification preferred. Must obtain the certification within six months of hire date.
- For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
