About the Role

Join a successful and growing fast-paced Hospice Agency that desires your help. We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience.

Responsibilities

• Provide emotional support, helping patients live their final days with dignity and comfort
• Participate and provide feedback to the team for regular interdisciplinary group meetings
• Develop, review and revise the patient/family plan of care
• Adhere to AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervise paraprofessionals providing services to patient
• Prepare clinical and progress notes, participate in IDG conferences
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communicate information using current process and technology within organization

Requirements

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• An empathetic approach to patients and caregivers
• Ability to adapt to patient's status and needs on a full-time, on-call basis
