Hospice Licensed Clinical SW - Social Services
Coordinates psychosocial care and services for assigned hospice patients in collaboration with the patient, family, physician, patient care team, and payers. Utilizes advanced psychosocial skills to facilitate care through assessment, planning, intervention, and evaluation. Promotes effective resource utilization and serves as a role model to achieve optimal clinical and resource outcomes.
Responsibilities
- Completes psychosocial assessments to screen for patients/families at risk for crisis, abuse, financial, or environmental issues; collaborates with the interdisciplinary team to identify needs.
- Acts as a facilitator on complex cases; makes Department of Children and Families (DCF) referrals for suspected safety, neglect, potential abuse, or complex family issues.
- Recommends supportive and therapeutic interventions to ensure a safe and efficient plan of care and quality patient outcomes.
- Identifies and documents necessary transitional care needs (Assisted Living Facility, Long-Term Care facility, respite, and transfer to inpatient level of care).
- Collaborates to determine desired goals and communicates all options to patients, families, Licensed Practitioners, and staff members.
- Identifies "at risk" patient populations and assesses strengths of patients and family support systems.
- Assesses safety planning, including suicide ideation and crisis Baker Act situations.
- Reinforces and documents education with patient/family on psychosocial aspects of care and expected outcomes.
- Initiates appropriate referrals to Ethics Committee, Risk Management, or Legal services as needed.
- Participates in resolution of legal issues such as guardianship, power of attorney, etc.
- Assists with the implementation of advanced directives.
Requirements
- Master’s degree in Social Work (MSW).
- Three (3) years of previous experience in Social Work.
- Clinical Social Worker licensure in the State of Florida.
- Valid Driver’s License and clean Motor Vehicle Report (MVR).
- American Heart Association (AHA) CPR AED HeartSaver certification (prior to start date and maintained).
- Must satisfy Florida Agency for Health Care Administration (ACHA) Level two (II) background screening.
Preferred Qualifications
- Five (5) years of previous experience in Social Work in an Acute Care, Hospice, or Home Care setting.
- Proficient in Microsoft Office – Outlook, Word, Excel, PowerPoint.
Skills & Knowledge
- Maintains current knowledge and awareness of payer and reimbursement practices, and regulations impacting the patient's plan of care.
- Demonstrates professional accountability through supporting patient’s rights, informed consent, and advanced directives.
Physical Requirements
- Physically agile; may require frequent walking, standing, running, bending, stooping, kneeling, climbing, or crouching.
- May occasionally lift or move objects up to 50 pounds, with or without assistance.
- May require moving in different positions to accomplish tasks in various environments, including tight and confined spaces, reaching out and above shoulders, or overhead.
- May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift.
- May be exposed to inside and outside environments with varied temperatures, air quality, lighting, and/or low to loud noise.
- May work with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines.
- Requires communicating with others to exchange information, visual acuity, and hand-eye coordination.
- Workspace may vary from open to confined.
- May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
