Key Responsibilities
- Receive and process new patient referrals from hospitals, physicians, and other healthcare providers.
- Conduct initial eligibility screening for hospice services.
- Schedule admissions visits with clinical teams.
- Collect and verify patient demographic, insurance, and clinical information.
- Enter all referral and patient data accurately into Electronic Health Records (EHR) or hospice software (e.g., MatrixCare).
- Communicate with families and caregivers regarding hospice services, benefits, and admission process.
- Cookordinate with intake and clinical staff to ensure timely and compliant admissions.
- Maintain accurate and up-to-date tracking logs and reports for all referrals and admissions.
- Support billing and compliance by ensuring required documents are completed before patient start of care.
- Provide exceptional customer service and maintain professional communication with external and internal stakeholders.
Qualifications
- A high school diploma is required; an associate’s or bachelor’s degree is preferred.
- Experience in healthcare intake, hospice, home health, or case management is preferred.
- Strong organizational, multitasking, and communication skills are essential.
- Proficiency in Microsoft Office and EHR software is required.
