Hospice Homecare CNA - Day Shift (East Madison Area)
About the role
The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.
Responsibilities
- Assist Patients with Activities of Daily Living
- Ambulate, change position, and move to/from sitting and standing
- Help with toileting, incontinence care, bathing, showering, dressing, oral care, grooming, eating, and drinking
- Perform post mortem care
- Obtain vital signs
- Assist with treatments
- Anticipate and respond to patient needs
- Provide homemaker assistance
- Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash
- Order and deliver patient supplies
- Help prepare simple meals
- Communicate by reading, understanding, and following the plan of care
- Document care and changes in patient condition in electronic health record
- Document and report significant changes in condition or unusual incidents to appropriate team members
- Reinforce patient, family, and facility partner education
Requirements
- High school diploma or GED, or equivalent combination of education and work experience
- Graduation from an accredited Certified Nursing Assistant program
- Current, unrestricted Nursing Assistant Certification in state of employment
- One-year personal care experience, hospice, or home health, preferred
- Valid driver’s license, auto insurance, and access to a reliable vehicle
- Basic Microsoft Office skills
- Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
- Ability to use proper body mechanics, safe patient handling, and standard precautions

Pay
Starting Pay: $18.50 + (based on experience)
Schedule
Full-time (1.0 FTE) 40 hours/week | Day Shift | Monday-Friday | 8:00am-4:30pm Includes Weekend (every 12th) and Holiday (3/year) rotations
