Hospice / Home Health Nurse Weekend

About the role

This position provides after hours / weekend on-call support for home health and hospice patients in the Portland metro area. Hours are flexible. Position can be part-time or full-time.

As a Hospice Nurse, you’ll be delivering person-centered care to patients in their time of need. Hospice is not about giving up—it’s about being pain free and comfortable, and maximizing every moment.

Responsibilities

• Triage calls for AgeRight Hospice and Home Health after hours and on weekends and perform patient visits as needed
• Provide education to patients and families about end of life care
• Perform end of life care and support to families
• Provide care to home health patients
• Perform weekend admissions as needed

Schedule

• Thursday 3 pm – Friday 8 am
• Friday 3 pm – Monday 8 am

Qualifications

• Licensed RN in good standing in the State of Oregon
• Experience working in Hospice required
• Valid driver’s license and proof of insurance
• Dependable transportation
