Hospice Home Health Aide

About the role

The Home Health Aide (HHA) position works under the direction of a Case Manager/DON and is responsible for providing resident care to assigned residents in accordance with resident assessments, care plans and as directed by nursing management. The HHA works with other licensed care staff to coordinate the proper types of care to residents and strictly follows all Company nursing policies and procedures as well as safety guidelines.

Responsibilities

• Work closely with patients on a 1-1 level while getting strong support from a patient care team
• Collaborate daily with nurses, social workers, chaplains, volunteers, and HHA leadership

Requirements

• Home Health Aide certification in the state(s) of practice
• CPR certified
• Must be willing to drive with reliable transportation, a valid driver's license, and auto insurance
• Able to work independently on a day-to-day basis
• Ability to work well within a team environment
• Strong commitment to improving quality of life while partnering with community organizations in patient care
• Eager to gain higher level opportunities such as certifications and further education

Pay Scale $20-23 per hour
